COMMUNITY ASSESSMENTS

Objective assessment where everyday function happens

Our occupational therapists provide comprehensive assessments in the home, workplace and community to understand how an individual’s abilities relate to the demands of everyday activities.

Assessments combine interview, clinical observation and functional testing to provide practical recommendations regarding safety, independence, equipment, assistance, accommodation and rehabilitation.

Occupational Therapy Home Assessment

A Home Assessment evaluates how safely and independently an individual is able to manage everyday activities within their home.

The occupational therapist assesses relevant physical, cognitive and functional abilities and may observe the client completing activities such as mobility, transfers, personal care, meal preparation and household tasks.

Recommendations may include equipment, environmental modifications, compensatory strategies, rehabilitation or assistance with daily activities.

Personal Care Assessment

A Personal Care Assessment evaluates an individual’s ability to safely and independently complete personal and instrumental activities of daily living.

The assessment considers what the client can complete independently, where assistance is required and how frequently support may reasonably be needed.

Recommendations are based on interview, clinical assessment and observation of functional abilities and may address personal care, household activities, meal preparation and other daily needs.

Physical Demands Analysis

A Physical Demands Analysis (PDA) objectively documents the physical and environmental demands of a job.

The therapist observes the work and quantifies demands such as lifting, carrying, pushing, pulling, reaching, postures, mobility and repetition.

A PDA can assist with return-to-work planning, job matching, accommodation recommendations, Functional Capacity Evaluation comparisons and injury prevention.

Ergonomic & Worksite Assessments

Ergonomic assessments identify physical and environmental factors that may contribute to discomfort, injury or difficulty performing work.

The occupational therapist evaluates the worker, workstation, equipment and job demands and provides practical recommendations aimed at improving positioning, reducing unnecessary physical demands and supporting safe work performance.

A written report outlining findings and recommendations can be provided.

Computer Ergonomics Training

Interactive workplace education is available for individuals or groups and can include workstation setup, posture, equipment positioning, common ergonomic risk factors and practical strategies employees can apply throughout the workday.

Permanent Impairment Assessment

Permanent impairment assessments provide objective measurement and documentation of relevant physical findings for impairment determination.

Assessment may include range-of-motion measurement and evaluation of scarring depending on the purpose of the referral.
